This form is used only if you are moving out of the Marshfield Utilities service area and are
closing your account. You can also terminate service by calling our office at (715) 387-1195 and requesting a form be
mailed to you. When you submit this completed form, we will schedule the final meter reading.
One business day is required to terminate service and the final bill will be prepared within
15 days of termination.
